PR#246: docs: Consolidate project docs into Sphinx docs -

This commit does the following:

Converts contributing and maintainer guidelines from hidden Markdown
files into existing Sphinx documentation
Reorganize docs index page to add multiple categories
Update README to point to new URLs for contributing guidelines

I tested this locally in my environment and confirmed the docs render as
expected.
