Add VLV indices and pagination support to authority list API
and CLI.

Lightweight CA should be covered as a new feature on Idm side

I'm inclined to close this. It is not expected that deployments would use huge numbers of LWCAs such that pagination would be necessary. If someone really needs it in the future we can re-open.
