When cloning, it is possible to set up the database and replication agreements ahead of time
(see pki_clone_setup_replication). Part of the preparation of the clone DB might include creating the indexes and vlv indexes.

We should provide a script to allow folks to do that easily. The most likely solution is to do this as part of pki-server.
