#248 Update entries midway refresh to be relative to the expiration date

Right now we provide a fixed value in seconds after which we consider an entry for midway refresh.
That value should instead be a percentage relative to the expiration date/last update date delta.
So setting the value to 0.5 should always refresh when:
cur date > last update + (exp. date - last update)/2

Probably internally we should also ignore midway refreshes if it turns out that (exp. date - last update) is a very small number (like 10 seconds or so).


Moving this to 1.0 RC, as it is a config file change.
