This PR adds two examples of a completed logic model (Diversity FAD 2017, CommOps FAD 2018) and links to them in the README. Ideally, this helps someone see them used in practice too.

We can add in @riecatnor's updates once we get a chance to circle back. But might as well include some examples for now.
