Create a new issue template for this repo to guide someone when writing a Code of Conduct-related ticket
A template with helpful questions to guide someone's response in a CoC-related ticket is helpful for the person filing a ticket and for the Council in reviewing the issue.
For the person filing the ticket, it is easier to structure feedback that may be charged with emotion. If given a wide open form, it's harder to prescribe what the problem is. I believe it puts more pressure on non-native English speakers to clearly explain why they believe behavior is problematic.
For the Council, it makes it easier to do the following:
The options to be creative are difficult when you're limited to plain text, but a questionnaire-like template may be most effective. There should be simple yes/no or multiple-choice questions to help triage the type of issue the person is reporting first. Then, I believe wide, open-ended questions to loosely guide the filer's complaint are best to give the person an opportunity to explain the issue in greater detail.

This was closed without context due to a posting error. The context is that these ideas are being considered along with the new CoC work. That work includes a template. Sorry we didn't notice that the text failed to post.
